The Community Health Inclusion Sustainability Planning Guide (CHISP) was created as part of a collaboration between NCHPAD, Easter Seals and other national partners to provide inclusive guidance to local health leaders. The CHISP is intended to help create and enhance local health coalitions, as well as to provide guidance for incorporating elements of sustainable inclusion into existing and upcoming health promotions and efforts. It is based on and intended to be used in tandem with the Centers for Disease Control and Prevention’s (CDC) A Sustainability Planning Guide for Healthy Communities, with each section of the CHISP mirroring the Planning Guide and adding sustainable inclusive strategies and examples to ensure health promotion efforts are inclusive of individuals of all ability levels from planning to execution to evaluation
